Washed red blood cells

Red blood cell concentrates are washed to significantly reduce the amount of various plasma proteins or other substances that may cause adverse reactions in rare situations. Washing the product can result in a loss of up to 25% of the red blood cells in the original product, which reduces the expected increase in hemoglobin concentration after transfusion. Washing removes almost all other residual substances (e.g., anticoagulant, platelets, leukocytes, and potassium).

The washing method used at Héma-Québec is carried out in a closed circuit, and the suspension medium is an injectable solution of AS-3 (additive) used as a preservative.


Action

The post-transfusion function of washed red blood cells is similar to that of packed red blood cells in AS 3/SAGM.


Main indications

Washed red blood cells are indicated for patients who have already experienced at least one major allergic reaction after transfusion of labile blood products. Consult the MSSS Recommendations on the Use of Washed Packed Red Blood Cells (Recommandations sur l’utilisation de culots globulaires lavés – in French only).


Contraindications

The contraindications are the same as those of AS-3/SAGM red blood cell concentrates.


Dosage and administration

The dosage and method of administration are the same as for AS-3/SAGM red blood cell concentrates.

The transfusion must be completed within 4 hours of the product leaving the controlled environment.


Storage

Red blood cells washed in a closed circuit have a shelf life of 7 days in AS-3 when stored between 1 and 6°C, unless irradiated: refer to the label on the bag.
